    An unusual presentation of acute lymphoblastic leukaemia with peri-cardial effusion causing cardiac tamponade

    Peri-cardial effusion is most commonly associated with tuberculous infection in the developing world. Peri-cardial effusion causes symptoms when it is large or when it has accumulated rapidly. Non-tuberculous causes of peri-cardial effusion include bacterial infections, uraemia, viral infections, rheumatic fever, connective tissue disorders, post – peri-cardiotomy syndromes and malignancy.     Key factors affecting performance of biogas latrines in urban informal areas: Case of Kampala and Nairobi, East Africa

    Large scale application of biogas latrine technology in developing countries faces technical, socioeconomic and financial  challenges. As a result, harnessing its full potential has not been realized. This study examined variables describing the design, construction, operation and maintenance of nineteen biogas latrines in  relation to their performance in Kampala and Nairobi, based on survey and field observations. Pre-tested questionnaires were administered to users, owners and construction  technicians/masons of the biogas latrines. Field observations were also undertaken to assess physical conditions of the biogas latrines. Principal component analysis was then used to establish correlation between variables of design, construction, operation and maintenance in relation to the performance of biogas latrines in terms of burning hours in a day. The design types of all the studied biogas latrine digesters were found to be of fixed dome. Co-digestion of human excreta and cow dung increased the number of biogas burning hours in a day from 0.5 to 1.1. The findings also show that the performance of the biogas latrines was influenced by six of the variables examined describing construction, operation and maintenance: skills of masons, use of standards in construction, training of users on operation and maintenance aspects, number of users/owners and their motivation for installation of biogas plants and physical conditions of the biogas latrines. This  implies that the use of skilled masons,  comprehensive training of users on operation and maintenance aspects and use of cosubstrates are key variables for optimal performance of biogas latrines.Key words: Biogas latrines, Kampala, Nairobi, performance

    Western Indian Ocean marine and terrestrial records of climate variability: a review and new concepts on land-ocean interactions since AD 1660

    We examine the relationship between three tropical and two subtropical western Indian Ocean coral oxygen isotope time series to surface air temperatures (SAT) and rainfall over India, tropical East Africa and southeast Africa. We review established relationships, provide new concepts with regard to distinct rainfall seasons, and mean annual temperatures. Tropical corals are coherent with SAT over western India and East Africa at interannual and multidecadal periodicities. The subtropical corals correlate with Southeast African SAT at periodicities of 16–30 years. The relationship between the coral records and land rainfall is more complex. Running correlations suggest varying strength of interannual teleconnections between the tropical coral oxygen isotope records and rainfall over equatorial East Africa. The relationship with rainfall over India changed in the 1970s. The subtropical oxygen isotope records are coherent with South African rainfall at interdecadal periodicities. Paleoclimatological reconstructions of land rainfall and SAT reveal that the inferred relationships generally hold during the last 350 years. Thus, the Indian Ocean corals prove invaluable for investigating land–ocean interactions during past centuries

    MEETING REPORT: UNESCO-MERCK AFRICA RESEARCH SUMMIT 2015- ACCELERATING ACCESS AND SUSTAINING INNOVATION 'FROM AFRICA FOR AFRICA'.

    Background: The Ebola virus disease outbreak of 2014 was the largest, longest and most devastating in the history of the disease. It demonstrated the social and economic impact an emerging infectious disease can have in a globalized world. Health systems in affected countries were stretched to the point of near collapse, while social relations and traditional practices were negatively impacted. Heads of African research institutions, African government representatives, leaders of global pharmaceutical companies, global infectious disease experts and close to 100 young African researchers from 25 countries; Assembled in Geneva on 19 and 20th October 2015, for the inaugural UNESCO-Merck Africa Summit sponsored by the United Nations Educational, Science and Culture Organization and Merck KGA Goal of Summit: The primary goal of the summit was to develop strategies to increase health research capacity in Africa, with special focus on Ebola and enhancing pandemic preparation for emerging infectious diseases. The summit was also provide a forum to showcase the research taking place in Africa, and provided platform for African researchers to network. Some of the key issues discussed included; strategies for enhancing policy frameworks to promote knowledge translation, strengthening of health systems, enhancing knowledge and data sharing, and increasing innovation in Africa. Conclusions: Summit attendees recognized that Africa still bore the heaviest burden of infectious disease, and increased commitment by African governments to fund health research, offered the best hope for developing health solutions and interventions to improve the health of Africans.     CPD1 (also known as ANP32-E) belongs to a family of evolutionarily conserved acidic proteins with leucine rich repeats implicated in a variety of cellular processes regulating gene expression, vesicular trafficking, intracellular signaling and apoptosis. Because of its spatiotemporal expression pattern, CPD1 has been proposed to play an important role in brain morphogenesis and synaptic development.We have generated CPD1 knock-out mice that we have subsequently characterized. These mice are viable and fertile. However, they display a subtle neurological clasping phenotype and mild motor deficits.CPD1 is not essential for normal development; however, it appears to play a role in the regulation of fine motor functions.     High-throughput Screening and Sensitized Bacteria Identify an M. tuberculosis Dihydrofolate Reductase Inhibitor with Whole Cell Activity

    Mycobacterium tuberculosis (Mtb), the causative agent of tuberculosis, is a bacterial pathogen that claims roughly 1.4 million lives every year. Current drug regimens are inefficient at clearing infection, requiring at least 6 months of chemotherapy, and resistance to existing agents is rising. There is an urgent need for new drugs that are more effective and faster acting. The folate pathway has been successfully targeted in other pathogens and diseases, but has not yielded a lead drug against tuberculosis. We developed a high-throughput screening assay against Mtb dihydrofolate reductase (DHFR), a critical enzyme in the folate pathway, and screened a library consisting of 32,000 synthetic and natural product-derived compounds. One potent inhibitor containing a quinazoline ring was identified. This compound was active against the wild-type laboratory strain H37Rv (MIC99 = 207 µM). In addition, an Mtb strain with artificially lowered DHFR levels showed increased sensitivity to this compound (MIC99 = 70.7 µM), supporting that the inhibition was target-specific. Our results demonstrate the potential to identify Mtb DHFR inhibitors with activity against whole cells, and indicate the power of using a recombinant strain of Mtb expressing lower levels of DHFR to facilitate the discovery of antimycobacterial agents. With these new tools, we highlight the folate pathway as a potential target for new drugs to combat the tuberculosis epidemic

    Climate fluctuations of tropical coupled system: The role of ocean dynamics

    The tropical oceans have long been recognized as the most important region for large-scale ocean–atmosphere interactions, giving rise to coupled climate variations on several time scales. During the Tropical Ocean Global Atmosphere (TOGA) decade, the focus of much tropical ocean research was on understanding El Niño–related processes and on development of tropical ocean models capable of simulating and predicting El Niño. These studies led to an appreciation of the vital role the ocean plays in providing the memory for predicting El Niño and thus making seasonal climate prediction feasible. With the end of TOGA and the beginning of Climate Variability and Prediction (CLIVAR), the scope of climate variability and predictability studies has expanded from the tropical Pacific and ENSO-centric basis to the global domain. In this paper the progress that has been made in tropical ocean climate studies during the early years of CLIVAR is discussed. The discussion is divided geographically into three tropical ocean basins with an emphasis on the dynamical processes that are most relevant to the coupling between the atmosphere and oceans. For the tropical Pacific, the continuing effort to improve understanding of large- and small-scale dynamics for the purpose of extending the skill of ENSO prediction is assessed. This paper then goes beyond the time and space scales of El Niño and discusses recent research activities on the fundamental issue of the processes maintaining the tropical thermocline. This includes the study of subtropical cells (STCs) and ventilated thermocline processes, which are potentially important to the understanding of the low-frequency modulation of El Niño. For the tropical Atlantic, the dominant oceanic processes that interact with regional atmospheric feedbacks are examined as well as the remote influence from both the Pacific El Niño and extratropical climate fluctuations giving rise to multiple patterns of variability distinguished by season and location. The potential impact of Atlantic thermohaline circulation on tropical Atlantic variability (TAV) is also discussed. For the tropical Indian Ocean, local and remote mechanisms governing low-frequency sea surface temperature variations are examined. After reviewing the recent rapid progress in the understanding of coupled dynamics in the region, this study focuses on the active role of ocean dynamics in a seasonally locked east–west internal mode of variability, known as the Indian Ocean dipole (IOD). Influences of the IOD on climatic conditions in Asia, Australia, East Africa, and Europe are discussed. While the attempt throughout is to give a comprehensive overview of what is known about the role of the tropical oceans in climate, the fact of the matter is that much remains to be understood and explained. The complex nature of the tropical coupled phenomena and the interaction among them argue strongly for coordinated and sustained observations, as well as additional careful modeling investigations in order to further advance the current understanding of the role of tropical oceans in climate
